For the Ladies:

A necklace she has always wanted
Set up a cute dinner at home for the two of you
Write her a song
BIG stuffed animal with a love note
Chocolate of course
Mix CD with the songs that remind you of her
Searching the store for the BEST card
Picnic in the park on a pretty day
Every girl loves flowers
A whole day dedicated to what she would like to do

For those Gent's:

Some type of yummy dessert baked by YOU!
Picture of you two in a cute handmade frame
Mix CD with all those love songs you listen to everyday
Huge Handmade card filled with things he loves
Small stuffed animal long with special note
Those things he just won't buy for himself. (Belts, Watches, etc.)
Lots of candy
A long meaningful note telling him how much you like him and how much he means to you.
Participate in something he loves to do, but you...eh not so much.
And if he is just a simple guy, gift cards to his favorite store always work.
